Primary Responsibilities:
  • Provide day-to-day oversight of case management staff and coordination of care for high risk senior Medicare Advantage and D-SNP population
  • Actively evaluates caseloads and appropriate distribution of work
  • Monitor employee performance to maximize efficiency and performance
  • Supervise staffing needs as indicated by Director and Clinical Manager
  • Assist the Director and Clinical Manager in developing, implementing and monitoring the quality assurance process 
  • Identify compliance and quality improvement opportunities and initiate corrective actions 
  • Assist in implementing procedures to provide excellent customer service to members through communication, staffing, phone availability and service execution 
  • Identify issues, training needs, and develop processes within the Case Management department 
  • Conduct performance reviews and evaluations of staff

Required Qaulifications:

  • RN with a current, unrestricted license in the State of CA
  • Bachelor’s degree nursing (BSN) a plus
  • Computer proficiency in MS Office: Outlook, Word, Excel, PowerPoint 
  • Excellent written and verbal communication skills

Preferred Qualifications:
  • 2+ years of Case Management experience
  • Knowledge/experience with Accountable Care Organization (ACO)
  • 2+ years of Supervisory/Management experience
  • Certified Case Manager (CCM)
  • 2+ years of Managed Care/IPA/Health plan experience
